Why vet costs vary 5-fold across countries
In 2026, a Maine Coon owner in San Francisco walks out of a 24-hour ER with a $4,200 bill for ureteral obstruction surgery. The same operation in Berlin costs €2,800; in Mumbai <₹85,000 (~$1,020). The procedure is structurally identical — what differs is the cost of veterinary education, real estate, equipment, drug pricing, and effectively the regulated fee floor.
Germany has the most transparent system. The GOT (Gebührenordnung für Tierärzte) is a federally regulated fee schedule that sets minimums and maximums for every procedure. The November 2022 amendment lifted rates 19% overnight and locked in 1×-4× multipliers for emergency vs routine work. Patients pay more but always within a defined band — BTK enforces.
The US has no federal vet fee schedule. AVMA publishes voluntary survey data, but every clinic prices independently. AVMA's 2025 figures show median dental cleaning $650 nationwide, with metro markets (SF, NYC, Seattle) at 1.5-1.8× the median. ER hospital networks like BluePearl, VCA and IVC Evidensia consolidated 40%+ of US specialist capacity by 2024 — broadly raising the floor.
UK pricing came under the CMA microscope in 2024 when the Competition and Markets Authority opened an investigation into the rise of corporate-owned vet practices (CVS, IVC, Pets at Home Vet Group, Medivet). The interim report flagged a 47% real-terms price rise between 2015 and 2023 and recommended structural reform. PDSA, Blue Cross, RSPCA charities continue to provide subsidised care for eligible owners.
Indian vet pricing reflects three structural realities: high labour-cost efficiency, generic drug availability, and an under-supplied specialist market. Cessna Lifeline (Bangalore), Max (Delhi), CGS (Pune) charge roughly 2× tier-2-city rates because they have the only MRI machines for hundreds of kilometres. Insurance penetration is low (under 2%) which keeps cash payments dominant.
